ABSTRACT

Objective: To observe the clinical efficacy of thunder-fire moxibustion for hypomenorrhea after induced abortion. Methods: A total of 74 patients with hypomenorrhea after induced abortion were randomized into two groups, with 37 cases in each group. The control group was treated with conventional hormone sequential therapy, and the observation group was treated with thunder-fire moxibustion. The course of treatment was 3 menstrual cycles. The traditional Chinese medicine (TCM) symptom scores, menstrual volume, endometrial thickness and the sex hormone levels were observed before and after treatment. The clinical efficacy was compared after treatment. Results: The total effective rate in the observation group and the control group was 91.4% and 73.5%, respectively, and the difference between the two groups was statistically significant (P<0.05). After treatment, the TCM symptom scores in both groups decreased significantly (all P<0.05), and the scores in the observation group were lower than those in the control group (P<0.05). The menstrual volume, endometrial thickness and each sex hormone level of the patients in both groups were significantly improved after treatment (all P<0.05), and the improvements in the observation group were more significant than those in the control group (all P<0.05). Conclusion: Thunder-fire moxibustion can significantly improve the clinical symptoms of patients with hypomenorrhea after induced abortion; it can increase menstrual volume and endometrial thickness, and regulate the sex hormone levels, producing a better curative effect than the conventional hormone sequential therapy.

ABSTRACT

Objective To investigate the hyperglycemia rate and its correlative factors for the undiagnosed diabetes population in Xuhui District of Shanghai, and to provide the basis for early intervention of diabetes mellitus. Methods Multi-stage cluster sampling was adopted by probability proportionate to size method, and physical examination information and questionnaires were made and collected from 10 130 and 9085 people in 2010 and 2015 respectively from 5 000 households, blood-glucose being examined at the same time.Univariate analysis was made by t test and χ2 test and logistic regression was used for analysis of related factors. Results In 2010 and 2015, the diabetes incidence rate was 2.21% and 1.93% respectively, and the hyperglycemia incidence rate in 2015 was higher than that in 2010 (15.28% vs. 12.63%), both increased with age and BMI value. The rate was higher in those with lower education, abnormal blood pressure, self-reported hypertension, self-reported coronary heart disease and smokers. The logistic regression analysis results showed the incidence of diabetes was positively correlated with age, BMI, abnormal blood pressure and smoking, while the incidence of hyperglycemia was positively correlated with the survey year, sex, age, BMI and abnormal blood pressure; both were negatively correlated with educational level. Conclusion The hyperglycemia incidence rate is high among the community undiagnosed diabetic population. We should focus on high-risk screening for diabetes mellitus, and key intervention of the population with low educational level, overweight and obesity, hypertension and unhealthy lifestyle.

ABSTRACT

Objective: To observe the clinical efficacy difference in treating irritable bowel syndrome-diarrhea (IBS-D) of liver-qi stagnation and spleen-deficiency pattern with different treatment protocols, and the effects on serum levels of 5-hydroxytryptamine (5-HT), substance P (SP) and vasoactive intestinal peptide (VIP), for unveiling the mechanism of intradermal needle therapy plus pinaverium bromide in treating IBS-D. Methods: A total of 123 IBS-D patients were divided into an observation group, a Western medication group and an integrated Western and Chinese medication group using the random number table method, with 41 cases in each group. The Western medication group was given oral pinaverium bromide, 50 mg each time and 3 times a day. The integrated Western and Chinese medication group was given additional Chinese herbal medicine Tong Xie Yao Fang, one dose each day. The observation group was given additional intradermal needle therapy on the basis of the Western medication group. The whole intervention lasted for 6 weeks. Before and after treatment, the scores of gastrointestinal symptoms, traditional Chinese medicine (TCM) symptoms, irritable bowel syndrome (IBS) symptom severity scale (IBS-SSS) and IBS quality of life (IBS-QOL) questionnaire, as well as the serum levels of 5-HT, SP and VIP were observed. The clinical efficacy was estimated. Results: The total effective rate was 92.7% in the observation group, 68.3% in the Western medication group and 78.1% in the integrated Western and Chinese medication group. The total effective rate was higher in the observation group than in the other two groups, and higher in the integrated Western and Chinese medication group than in the Western medication group, showing statistical significance (all P<0.05). After treatment, the scores of gastrointestinal symptoms, TCM symptoms and IBS-SSS showed significant decreases in the three groups, presenting statistical significance compared with the baseline (all P<0.05); the scores of gastrointestinal symptoms, TCM symptoms and IBS-SSS were notably lower in the observation group than in the other two groups (all P<0.05), and lower in the integrated Western and Chinese medication group than in the Western medication group (all P<0.05). After treatment, the eight component scores of IBS-QOL showed significant increases in the three groups compared with the baseline (all P<0.05); the eight component scores in IBS-QOL were significantly higher in the observation group than in the other two groups (all P<0.05), and higher in the integrated Western and Chinese medication group than in the Western medication group (all P<0.05). After treatment, the serum levels of 5-HT, SP and VIP decreased markedly in the three groups compared with the baseline (all P<0.05); the serum levels of 5-HT, SP and VIP were significantly lower in the observation group than in the other two groups (all P<0.05), and lower in the integrated Western and Chinese medication group than in the Western medication group (P<0.05). Conclusion: Treatment with intradermal needle therapy plus pinaverium bromide results in significant improvements in the gastrointestinal symptoms and quality of life in patients with IBS-D of liver-qi stagnation and spleen deficiency pattern, and effectively regulates the gastrointestinal hormone production.

ABSTRACT

OBJECTIVETo study the effect of maternal vitamin D deficiency on lung morphogenesis and platelet-derived growth factor-A (PDGF-A) expression in rat offspring.

METHODSSprague-Dawley (SD) female rats were randomly divided into two groups: normal control and vitamin D deficiency, with 6 rats in each group. The vitamin D deficiecy group was kept away from light and fed with the forage without vitamin D. After 2 weeks, the rats were mated with normal SD male rats. The morphological changes of fetal rat lungs on day 20 of gestation and 1-day-old neonatal rat lungs were observed by light microscope and electronic microscope. The levels of PDGF-A mRNA and protein in fetal and neonatal rat lungs were measured by reverse transcriptase-polymerase chain reaction (RT-PCR) technique and Western blot method respectively.

RESULTSUnder the light microscope, smaller alveolar space, smaller diameter of the respiratory membrane and thicker alveolus mesenchyma were observed in lungs of fetal and neonatal rats from the vitamin D deficiency group compared with the controls (P<0.05). Under the electronic microscope, fewer lamellar bodies but more glycogen deposition in intracytoplasm were observed in the lungs of fetal rats from the vitamin D deficiency group compared with the controls. There was an increased number of empty lamellar bodies in neonatal rats from the vitamin D deficiency group. The levels of PDGF-A mRNA and protein in lungs of fetal and neonatal rats from the vitamin D deficiency group were significantly lower than the controls (P<0.05).

CONCLUSIONSMaternal vitamin D deficiency during pregnancy may inhibit the development of lung morphogenesis and PDGF-A expression in late fetal and neonatal rats. The low expression of PDGF-A may be involved in the inhibitory effect of vitamin D deficiency on the lung development.

ABSTRACT

Objective To study the effect of different concentration of 1,25-dihydroxyvitamin D3[1,25(OH)2D3] on cell proliferation, differentiation and the expression of vitamin D receptor (VDR) in mouse MC3T3E1 osteoblast. Methods Osteoblast were cultured in medium with different concentrations of 1,25(OH)2D3.Incubated for 48 h,cell proliferation of osteoblast were examined by MTT reduction assay (mono-nuclear cell direc cytotoxicity assay),the osteocalcin (OC) levels in cell medium were detected by ELISA,and the expression of VDR mRNA and protein were examined by using SYBR Green real-time PCR and Western blot, respectively. Results 1.After incubation with 1,25(OH)2D3 for 48 h,the number of MC3T3E1 osteoblast was significantly less than that in control group(P0.05).3.SYBR Green real-time PCR and Western blot results showed that the expression of VDR mRNA as well as VDR protein of osteoblast in 10-8,10-9 mol/L experimental groups were significantly higher than those in control group (Pa0.05).Conclusions Cell proliferation of mouse osteoblast can be inhibited, while the cell differentiation was promoted by 1,25(OH)2D3.1,25(OH)2D3 up-regulated the expression of VDR in mouse osteoblast, which suggested that the VDR signal pathway may play some role in proliferation and differentiation of osteoblast.

ABSTRACT

Objective To explore the cataract surgery quality on blindness prevention and postoperative problems in village in short period. Design Population-based survey. Participants 251 cases(254 eyes) received operation and 131 cases(134 eyes)were surveyed 6-month postoperatively. Methods Patients were examined 6-month after the small incision extracapsular cataract extraction and intra ocular lens(IOL) implantation. Examinations were conducted by a special oculist including far vision, near vision, external in- spection, anterior segment, posterior segment, intraocular pressure. Main Outcome Measures Visual acuity, intraocular pressure, diopter, eye complications of surgery. Results Naked far vision of the operated eye more than or equal to 0.3 was 41.8%, naked far vi- sion of the eye more than or equal to 0.05 was 82.8%; corrected far vision more than or equal to 0.3 was 64.2%, corrected far vision more than or equal to 0.05 was 92.3%. Naked near vision more than or equal to 0.1 was 79.9%, corrected near vision more than or e- qual to 0.1 was 85.8%. The main postoperative complications were ametropia, posterior capsule opacification(PCO), deformed pupil, pupil displacement, pigments of IOL, eccentric IOL and intraocular hypertension. The chief reasons of eyes that could not be recovered were vitreous, retina or optic nerve diseases, the key factors that caused living vision less than 0.3 were ametropia, PCO, the disease of vitreous, retina and optic nerve. Conclusions The serious complications affecting the surgery result are limited in a low range. The most important factors of the eye corrected far vision less than 0.05 are the vitreous, retina and optic nerve diseases. In order to improve the visual sight, we should add equipment to calculate the IOL diopter accurately.
